The Shared Care Record is a secure, digital record that brings together key health and care information from the different organisations involved in your care — such as your GP, hospitals, community services, and social care teams.
It allows professionals to see a more complete picture of your health and wellbeing, helping them make safer and faster decisions about your care.
3. What Information We Share
The type of information shared through the Shared Care Record includes:
- Basic details such as your name, date of birth, address, and NHS number.
- Health and care information such as diagnoses, test results, medications, allergies, and treatment history.
- Social care information such as care plans, risk assessments, and support arrangements.
- Contact details for your next of kin or representatives.
Only the minimum information necessary to support your care is shared.
4. Why Your Information Is Shared
Sharing information helps us to:
- Provide safe, joined-up, and person-centred care.
- Avoid unnecessary repetition of information.
- Ensure the right professionals have the right information at the right time.
- Improve care planning and outcomes.
- Respond quickly in emergencies.
Your information is used only for your direct care.

Only authorised professionals directly involved in your care can access your Shared Care Record.

- Registered care providers such as The Old Coach House and Priory Care
All users have strict confidentiality agreements, access controls, and regular data protection training.
6. Lawful Basis for Processing
